The Sacrament of Baptism is the initial sacrament of faith. Through the waters of Baptism we enter a new kind of life, the life of the Spirit.

*Parents who have not yet had a child baptized need to attend a preparation class.  Please contact the Office to register for the class.  You may bring this form with you to the class.

*If you have already attended a class, please return this form in any of the following ways:
1. Email secretary@icjacksonmo.com
2. Mail it to the Church Office at 208 South Hope Street, Suite 102, Jackson MO
3. Drop it by the Church Office
4. Drop it in the white Drop Box located outside the Church Office
5. Drop it in the collection basket in an envelope clearly marked “Baptism Registration”
